What Documents Are Expected To Require A US Passport Renewal For Minor
Before we go into any depth on the documents required it is worth noting that a minor is any person under the age of 16. Although the process may state both parents or legal guardians there is a clause that allows for one parent to apply with or without the non-applying parents consent. This is a documentation and process guide on how parents or legal guardians should go about applying for a US passport passports for babies.
The first form which you must complete for a US online passport renewal is the application form itself, this is known as the application form for a US passport and its reference number is DS-11. Please remember though that this form should not be signed until you are instructed to do so by the acceptance agent. Another important point is that from the date on the form you have just five days to present the application to the US Department of State, after this time frame a new form will need to be completed.
Other required documents are, original proof of U. S citizenship such as the previous U. S passport or a birth certificate with a raised seal. A certified copy of a birth certificate is acceptable. Two identical photos are also required and should be 2 inches by 2 inches, the photo must be no older than six months and the background must be white.
If you are intending on leaving in two weeks or less you must attach an itinerary with the application, if this is missing the application may well be delayed. If both parents or legal guardians cannot be present with the applying minor then a notarized statement of consent or special circumstances form must be completed, a DS 3053. This allows one parents families the right to apply for a passport for a minor even if the non-applying parents cannot be contacted for consent. All of these documents will be checked and witness by the acceptance agent and sealed in the envelope.
Ensure you have the correct monies in the form of either checks or money orders and that they are payable to the U. S Department of State. Also remember to attach two signed letters of authorization, these can be signed by one party but you must have two copies.
Another form you must not forget is the passport processing order form and this will need to be attached to the sealed envelope. This is to aid the passport processing staff as it states clearly the name and date of both of the applicant as well as the departure date. So any urgent dates can be easily identified and dealt with immediately.
Make sure you have everything you need before you see the acceptance agent and whatever you do, do not forget the checks or money orders. With the application you will need to enclose the government fee and this will be sealed in the envelope. While other fees such as the service fee and FedEx return fee will need to be attached on the outside, the final fee is the acceptance agents fee.
Check and double check all documents and monies before your visit and if you are not 100% sure on anything contact your local acceptance officer or the issuing authority directly for clarification on your US online passport renewal. It is better to be doubly sure on the facts and figures before you leave rather than risk having a wasted journey. This may seem a lot to do but it is worth the effort, it safe guards your children and prevents anyone who is not authorized from obtaining a US expedite baby passport.
